One Care Portal builds software for agencies that deliver Medicaid-funded community care, such as waiver case management, targeted case management, support coordination, adult day care, IDD and supportive housing providers. Its case management product lets case managers write contact notes in the field and supervisors approve or return them. A compliance review product checks notes, charts and assessments against program rules and works with any EHR, while reviewers make the final call on each flag. A billing and revenue cycle product submits claims through a clearinghouse, posts 835 payments and helps staff work denials. Each product can be used on its own or combined under one monthly platform fee, with month-to-month contracts.
